Cilacar is actively composed of a calcium channel blocker called cilnidipine, which is approved for the treatment of high blood pressure (hypertension), chest pain (angina), and abnormal heart rhythms, such as atrial fibrillation. The medicine is also used for the treatment of other conditions such as pulmonary hypertension (high blood pressure in the lungs), Raynaud’s syndrome, and cardiomyopathy. Sometimes, Cilacar can be used to prevent migraine headaches. The drug cilnidipine was patented in 1984. It was approved for hypertension in 1995

Action Mechanism

Cilacar contains 10 mg cilnidipine and 40 mg telmisartan. Cilnidipine is a calcium antagonist, which means it inhibits the influx of calcium ions into the cells. This mechanism of action results in reduced vascular smooth muscle tone, dilating the blood vessels and leading to a reduction in high blood pressure. Telmisartan inhibits angiotensin II, a powerful vasopressor derived from angiotensin I. By doing so, telmisartan relaxes and dilates blood vessels so the blood can flow easily.

Dosage

  • Follow all directions as advised by your doctor before you start taking Cilacar T for treating high blood pressure.
  • Take this medicine orally with or without food, or as directed by your physician, usually once daily.
  • You must use this antihypertensive medication regularly to control your blood pressure and prevent serious cardiovascular events.
  • Make sure you take Cilacar T at the same time every day, without missing or skipping the dose. You must keep using it even if you feel well.
  • Do not chew, break, crush, or split the pill before oral consumption; otherwise, the drug may lose its efficacy.

Precautions

  • Before taking Cilacar T, inform your physician whether you have any drug allergies, especially cilnidipine or telmisartan, or any other CCBs or ARBs
  • Before using this high blood pressure medication, tell your physician about your medical history, especially liver disease, high blood potassium levels, dehydration, etc.
  • Cilacar T can make you a bit dizzy so stay away from risky activities like driving or operating machinery during treatment. Also, avoid alcohol during treatment.
  • Before undergoing any surgical procedures, inform your doctor about all the medications you have been using, including Cilacar T.
  • Cilnidipine and telmisartan could increase the risk of hyperkalemia (increased potassium levels). So, check with your doctor before using potassium supplements during treatment.

Warnings

  • Check with your doctor if you experience excessive sweating, vomiting, or diarrhea. Immediately see your doctor if there persistent vomiting or diarrhea.
  • Older people may be more susceptible to the side effects of Cilacar T, especially dizziness and urinary troubles in those who have chronic kidney disease.
  • Please note that Cilacar is not indicated to a pregnant woman pregnancy. That’s because the drug may harm the fetus, causing fetal issues.
  • Cilnidipine passes into breast milk but it is unknown whether telmisartan passes into breast milk. Nevertheless, consult with your doctor if you want to breast-feed during the treatment.
